Experiences of nurses in the use of medication cards at Bicol Medical Center, Naga City : a sub-system assessment towards patient safety

Abstract

Context: Patient safety means the patient is free of harm during the whole process of medication administration. The process starts from the transcription of a written order prescribed by the doctor, then progresses to the actual giving or administering of medication and documentation of procedure.

Study design: Descriptive method

Study setting: Bicol Medical Center, Naga City

Study population: The respondents to the questionnaires were the nurses in the patient care units of Bicol Medical Center using medication cards in the process of medication administration.

Results:
The findings were remarkable despite of the numerous and mandatory education regarding medication administration and even with the plethora of research on factors causing medication error. The extent of compliance to the application of the standard policy in medication administration was no completely evident at Bicol Medical Center. Safety indicators on conformity to noting medication order, checking transcription, observing standrad time, identifying patients and practicing the ten golden rules revealed inconsistencies in practice.
Conclusion:
The study concludes that the inaccurate use of medication card leads to unsafe medication administration, which may place the patient's safety at risk. The process of transcribing the same medication entry several times to different documents may cause confusion on the nurses performing the medication administration and may likely cause discrepancies and ineffective use of medication cards.
